The specific Home Health Care services provided are intermittent visits by the following:
MEDICARE COVERAGE GUIDELINES
Medicare requires that you meet the following criteria:
- You must be homebound. This means that due to your illness or injury there is an inability to leave the home and doing so would require a considerable and taxing effort. You can still be considered homebound if you:
- Leave your home infrequently or for a short duration.
- Receive medical treatment, therapeutic, or psychosocial activities in an accredited adult day care program.
- Leave your home only to attend religious services.
- Leave your home only to go to the beauty salon or barbershop.
- You must have had a recent illness or injury, which requires skilled nursing, physical therapy, and/or speech language pathology on an intermittent basis, or have a continuing need for occupational therapy.
- You must be under the care of a doctor who has ordered the treatment and services that we are providing. In order for Medicare to cover the services provided, they must be reasonable and necessary.
- A person with a psychiatric illness may be considered homebound even if there are no physical limitations. The nurse will explain specific criteria that apply to psychiatric diagnoses.
